Social Security Number

Treat as EIN or SSN

EIN

The number entered in the Social Security Number box is treated as an EIN.

SSN

The number entered in the Social Security Number box is treated as a SSN.

1099 or Employee

W-2

First Name

The employee’s first name to be printed on the Form W-2. If blank, Evolution® prints the employee name.

Middle Name

The employee’s middle name to be printed on the Form W-2. If blank, Evolution® prints the employee name.

Last Name

The employee’s last name to be printed on the Form W-2. If blank, Evolution® prints the employee name.

Name Suffix

The employee’s name suffix to be printed on the Form W-2. If blank, Evolution® prints the employee name.

Annual Form Type

Select the type of Form W-2:

 

Deceased

Yes

The Deceased check box is checked on the Form W-2.

No

The Deceased check box is not checked on the Form W-2.

Statutory Employee

Yes

If this employee is being paid with any pension or deferred compensation E/D code (code types DA through DK), Evolution® selects Yes. Therefore, Yes should only be selected if the pension is paid outside of payroll Act of paying individuals for services provided, including withholding taxes and other deductions. or uses an unusual E/D Code.

Pension

Yes

The Pension check box is checked on the Form W-2, which indicates that the employee is receiving pension or 401K benefits outside your company.

No

The Pension check box is not checked on the Form W-2.

 

If this Employee is being paid with any pension or deferred compensation E/D Code Type (codes DA through DK), Evolution® automatically checks the Pension box on the Form W-2. Therefore, Yes should only be selected if the pension is paid outside of payroll or uses an unusual E/D Code.

 

Residential State

If the Client has the flag Use State Reciprocate Laws on the W2 set to Yes at the Client level, then the state populates in the EE Residential State will be the state used to determine which reciprocating rules should be applied for W2 Reporting.

 

I-9 on File

Yes

Indicates that the Form I-9 (Employment Eligibility Verification ) is on file. The employee fills out the form within 3 days of hiring so the employer can verify eligibility of employment.

No

Indicates that Form I-9 is not on file.

1099R

Distribution Code

Type the distribution code assigned by the IRS. Evolution® prints this code in corresponding box on the Form 1099R.

Tax Amount Determined

Yes

The Taxable amount not determined check box is selected in box 2b on the Form 1099R.

No

The Taxable amount not determined check box is cleared in box 2b on the Form 1099R.

Total Distribution

Yes

The Total Distribution check box is selected in box 2b on the Form 1099R.

No

The Total Distribution check box is cleared in box 2b on the Form 1099R.

Pension Plan

Select the type of pension plan:
